Here are some additional spectacular bike path fails courtesy of the UK. The Brits seem to be utterly clueless on bike infrastructure. Strange considering they have several excellent examples of how to do it right just across the channel.

Or

A bike lane which you play Russian Roulette on the door zone, and then Completely ends and you fight to cross some defunct RR crossing whilst merging.

Wow, a textbook classic "door lane". Cycling on that road would be safer without the lane. With the lane there you're forced into a constant back-and-forth between the traffic land and the bike lane, usually you end up basically riding on the white line itself. So obvious that many of these lanes are designed by non-cyclists.

I think I recently saw a city planning document in which they said the lanes on Park and Portland would be moved to the correct, sane, (i.e. RIGHT) side of the street. Wish I'd saved that link.

If the Park and Portland left-side lanes are considered failures, why is Minneapolis still making left-side lanes? Off the top of my head: 6th Street downtown has a new left-side lane.

I have been wondering for a long time why they are putting lanes on the left side of one-ways. Whatever the reasons, they're wrong in my opinion.

I even see some cyclists ignoring these lanes and riding on the right where they feel safer.

Supposedly, a left-side bike lane decreases the potential for dooring (the driver would exit on the curb side of the street). Unfortunately, the passenger-side mirror is adjusted for the driver, so if there is a passenger exiting the vehicle, the likelihood that they would see a cyclists in the side view mirror is virtually nil.

You also have the added problem of drivers not expecting cyclists to be on the left side of the road (these left side lanes violate the principle of speed positioning - faster vehicles in the left lane) and therefore drivers are even less likely to check or cyclists before turning.

I actually really liked the counter-flow lanes on Marquette and 2nd Ave. before they got removed for the Marq2 project. Yes, the bikes were in an unexpected part of the road, but the visibility was good and I never had a problem with them. I don't like Park/Portland, though. I ride Chicago Ave. instead and avoid Park/Portland pretty much all the time these days.
